1、地点：成都市桐梓林北路1号-四川国际旅行卫生保健中心

4号线（9游会与尤文图斯达成官方合作站）—换乘1号线（骡马市）—桐梓林（B口出）

2、注意事项Notes：
（1）空腹，不吃早饭、不喝水 Do not eat and drink anything on the day.
（2）建议早上8周末不上班，点或更早出发，平时要在十点前到。Suggesting to go on 8 am or before. Only on weekdays, better arrive before 10 am.
（3）2寸证件照3张. Three 2-inch ID photo are needed.
（4）携带护照. Bring with passport.
（5）费用：508元左右，建议带600元。

（6）或者付邮费请医院邮寄。医院会给出发票和一张写有领取体检结果时间的单子，请按照单子上的时间领取体检结果，检查缴费结束后，
